Barrier, steel and formwork, in the hours before any of it becomes concrete.
The Marsden Park slab findings concentrate on the vapour barrier: not carried beneath the whole slab on one job, and punctures left unsealed on another. That layer only performs as one continuous membrane, so a gap at the edge and an untaped puncture amount to the same thing. Add cover below the minimum, beam widths varying and bar placement to check, and everything on the list stops being verifiable the moment the truck leaves.

Because the barrier works only as a continuous layer. A single unsealed puncture is a direct path for ground moisture into the slab, and it costs a strip of tape to close.
You can see the surface. Cover, the steel within and the barrier beneath are all beyond reach once concrete is placed.
